MY DETAILS?
If you change either your address
or telephone number please let reception staff know as soon as possible. If you
are currently attending hospital or are on any waiting list please let the hospital
know also, otherwise follow up appointments may go astray thus making the waiting
list even longer.

REGISTER?
If you wish to register with the
practice please come to reception and ask for details. You will be given a form
to complete and an appointment with a nurse for a new patient health check. Once
this has been completed we can then request your notes from the last practice
you were registered with. You are not assigned to a particular doctor but can see one
of your choice.

MAKE
AN APPOINTMENT?
We offer an appointment system and
you may see the doctor of your choice when available. Please make your appointment
in person or by telephone. Please cancel your appointment if you cannot attend. Failure to do so usually leads to patients having to wait longer than would otherwise
be necessary and also wastes valuable doctors' and nurses' time. It can also
mean delayed treatment for a patient presenting with an urgent problem. Urgent
cases will be seen on the same day wherever possible as will children under the
age of five years. Non-urgent appointments will be seen at the first available
time. Surgery times will vary when a doctor or nurse is away on holiday.

chaperone can be provided if required. Please ask about this at reception when
booking the appointment.

OBTAIN
A HOME VISIT?
These are for people who are too
ill or infirm to attend surgery. Please request home visits before 10.30am. Late
calls, unless they are emergencies may not be seen on that day. The receptionist
may ask for some details of your illness to help the doctor assess the urgency
of visits as they plan their rounds.

OBTAIN
A REPEAT PRESCRIPTION?
Prescriptions for medicines taken
regularly can be obtained without seeing a doctor. Requests can be made in person,
by post or by telephoning the surgery.
Requests by telephone can only be
accepted between 11.00am and 3.00pm. Please give 48 hours' notice to allow for
repeat prescriptions to be prepared. They are usually ready for collection after
1.00pm two days later. From time
to time you will be asked to see the doctor so that your medication can be reviewed.
Patients who require a regular supply of medication may well find it cheaper to
buy a prepayment certificate. Please ask at reception or your local chemist for
further details. If a prescription cannot be collected in person, please ensure
a stamped self addressed envelope is provided.

OBTAIN
A SICKNESS CERTIFICATE?
You do not need a sickness certificate
signed by your doctor for statutory sick pay unless you have been off work for
more than seven days. You may provide your employer with a self certification
note for the first seven days after which time you will need to consult your doctor
if you are unfit for work. If your employer asks for a sick note signed by your
doctor for the first seven days this is classed as a private sick note and therefore
a fee is payable. |
